Patient Rights and Responsibilities |
 | It is your right as a Patient: |
- To be informed of your rights in a comprehensive manner.
- To inform the patient and/or family about the care and services of the hospital and how to access those services.
- To have considerate and respectful care, consistent with your personal values and beliefs along with access to spiritual and religious support.
- To have care that is respectful towards your privacy.
- To provide the safeguarding of personal belongings during an emergency situation as per the Safety Security Policy.
- To be protected from any types of assault while a patient.
- To receive appropriate protection and services if you are a vulnerable child, disabled and/or elderly.
- To have all your information kept confidential and protected from loss or misuse.
- To have respectful and compassionate care if your condition is terminal.
- To have information on how to gain access to clinical research, investigation or clinical trials involving human subject (if this hospital is involved in any of these).
- To be informed on how you would be protected and communicated to, if you participated in any clinical research, investigations or clinical trials.
- To be informed on the hospital’s policy of complaints, conflicts and difference of opinions about patient care and your rights to participate in these processes.
- To expect staff to comply to your rights.
- Patient and Family have the right to receive a detailed explanation of their condition, plan of care during their in-patient visit and any change in the care plan. Also, the follow-up care plan should be communicated in a manner equivalent to the patients level of education and understanding.
- To give general consent for treatment and have the general scope and limits of this consent explained.
- To expect to be asked for an informed consent before surgery, anesthesia, use of blood and blood product, other high-risk treatment or procedure, clinical research, investigation and/or trial.
- To be informed of any conflicts of interest involving the health organization and your care.
- To participate in the care process.
- To expect information on how you and your family will be told about medical conditions and treatment and how to participate in care related decisions.
- To refuse or discontinue treatment.
- To have pain assessed and managed.

| It is your responsibility as a Patient: |
- To provide all personal and family health information needed to provide you with appropriate care.
- To participate, to the best of your ability, in making decisions about your medical treatment and to follow instructions and comply with the agreed upon plan of care. To acknowledge the consequences of not accepting care recommended from the provider or not following the plan of care.
- To inform your Physician and/or other care provider if you desire a transfer of care to another physician, caregiver or facility.
- To ask questions to your Physician or other care providers if you desire a transfer of care to another physician, caregiver or facility.
- To show respect and consideration of others receiving and providing care.
- To observe facility policies and procedures relating to smoking, noise, number of visitors and visiting hours.
- To accept any financial responsibility for health care services.
