| Discharge planning |
- The health care team will assess possible discharge needs from the time you arrive as a patient.
- Questions regarding your health care will be answered before discharge.
- Prescription and follow up appointments if necessary will be given on the day of discharge.
- Please arrange your own transportation home once the doctor has given you a discharge date.

| Non- Compliance with discharge planning : |
| When your doctor decides that you are ready to go home or to be transferred to another facility, you will be required to leave hospital as per the discharge date & time. Delaying your check out will affect other scheduled patients’ admissions/transfers or an emergency patient’s admission and therefore, it is our policy that appropriate action will be taken with patients who are not leaving the hospital as per the suggested time of discharge. |

| You will be informed of your time of discharge in advance. Be sure to take all personal belongings with you. Make sure that all discharge papers, appointments and medications are collected before you leave the hospital. |
