| It is required to have the following documents/information available at the time of registration and admission. |  |
- Original Insurance Card
- Passport or a true copy of passport
- Name and address of employer
- Name, address and telephone number of closest family member
- List of all medications you are currently taking and any medication allergies
- Attested marriage certificate for all expatriate patients requiring Obstetrics Services

| Parents or a legal guardian must be present to register their children below 21 years old. |
| Important Documents To Sign |
| You will be asked to sign the following documents at the time of admission |
- General admission, insurance & financial consent form
- Confidentiality & Authorization for the release of information form

| Our clerical associate will be assisting you in completing the admission procedure. |
| A wrist band printed with your identification information will be prepared immediately after the admission. A nurse will collect the necessary information from you about your general condition, diet, allergies, personal habits and other factors which is required during your hospitalization. |
